Bij Antes zoeken we een doortastende specialist ouderengeneeskunde die graag zijn of haar kennis en ervaring inzet binnen de dynamische wereld van de ouderenpsychiatrie en verslavingszorg. Dit ga je doen Samen met onze behandelaren, verpleegkundigen, psychologen en verslavingsartsen werk je aan begeleiding, diagnostiek en behandeling van ouderen met cognitieve klachten, psychiatrische of gedragsproblemen, lichamelijke aandoeningen en soms ook een verslaving. Een complexe, maar ongelooflijk waardevolle doelgroep waar je echt het verschil kunt maken. Bij ons krijg je veel regie, regelruimte en vertrouwen. Jij bepaalt in hoge mate hoe je jouw werk inricht, hoe je samenwerkt met je team en hoe je het behandeltraject vormgeeft. Zo kun je doen waar je goed in bent: kwalitatieve, mensgerichte zorg leveren met oog voor de hele persoon. De werkzaamheden: - je voert intakes bij cliënten thuis en stelt behandelplannen op; - je indiceert, coördineert en behandelt cliënten met complexe psychiatrische en somatische zorgvragen, vaak in combinatie met verslavingsproblematiek; - je neemt periodiek deel aan de spoeddienst tijdens kantooruren voor cliënten van de specialistische ouderenteams psychiatrie en geriatrie; - je onderhoudt actief contact met het netwerk en systeem van cliënten; - je draagt bij aan kennisontwikkeling en innovatie binnen het team; - je werkt outreachend en vertegenwoordigt het team in samenwerking met ketenpartners in de regio.
