In onze kleine teams werken we samen en houden we de lijnen kort, zodat jij je snel thuis voelt en direct impact kunt maken. De sfeer binnen onze organisatie is open en prettig; medewerkers werken graag bij ons en dat voel je! We beschikken over twee prachtige locaties, één in het hart van Amersfoort en één in Vathorst. Wat ga je doen? Je werkt binnen Zorg Thuis, waar zowel thuiszorg als verpleeghuiszorg wordt geleverd. Binnen de zorgverlening is sprake van gevarieerde en soms complexe zorgvragen. Naast uiteenlopende chronische ziektebeelden en palliatieve zorg is er regelmatig sprake van psychogeriatrische problematiek. Als EVV-VIG-er ben je verantwoordelijk voor het coördineren van zorg rondom een aantal cliënten en vervul je een belangrijke rol in de verbinding tussen cliënt, naasten, collega's en andere disciplines. Je: - bent verantwoordelijk voor het zo optimaal mogelijk ondersteunen van cliënten in hun kwaliteit van leven; - verzorgt en begeleidt cliënten conform het zorgleefplan; - coördineert de zorg rondom cliënten en onderhoudt contacten met naasten, behandelaren en andere betrokken disciplines; - stelt zorgleefplannen op, evalueert deze en zorgt voor passende bijstellingen; - signaleert veranderingen in de zorgvraag en vertaalt deze naar passende zorg en ondersteuning; - draagt actief bij aan de kwaliteit van zorg binnen het team; - denkt mee over verbeteringen in de dagelijkse praktijk en helpt deze ook te realiseren; - stimuleert eigenaarschap en professionele verantwoordelijkheid binnen het team...
