Dan voel jij je bij HWW zorg snel thuis. Als ergotherapeut binnen de Geriatrische Revalidatiezorg (GRZ) speel jij een belangrijke rol in het herstelproces van cliënten. Je werkt in een multidisciplinair behandelteam en begeleidt cliënten bij het herwinnen van hun zelfstandigheid na bijvoorbeeld een beroerte, operatie, val of andere acute gezondheidsproblemen. Jij maakt het verschil Als ergotherapeut GRZ help je cliënten om hun dagelijkse activiteiten zo snel en veilig mogelijk weer zelfstandig uit te voeren. Je kijkt niet naar beperkingen, maar naar mogelijkheden. Dankzij jouw deskundigheid kunnen cliënten met vertrouwen werken aan hun herstel en terugkeer naar huis. Je werkt nauw samen met specialisten ouderengeneeskunde, fysiotherapeuten, logopedisten, psychologen, diëtisten, verpleegkundigen en verzorgenden. Daarbij krijg je veel ruimte om zelfstandig invulling te geven aan jouw vak. Wat ga je doen? Als ergotherapeut GRZ: - behandel en begeleid je cliënten binnen de geriatrische revalidatiezorg; - onderzoek je de hulpvraag en stel je een behandelplan op gericht op een veilige en zelfstandige terugkeer naar huis; - train je cliënten in dagelijkse handelingen (ADL), mobiliteit en zelfredzaamheid; - adviseer je over hulpmiddelen, woningaanpassingen en voorzieningen; - begeleid en informeer je mantelzorgers bij het revalidatieproces; - werk je intensief samen binnen een multidisciplinair behandelteam; - draag je actief bij aan de kwaliteit van de behandeling en de verdere ontwikkeling van de GRZ; - leg je jouw bevindingen zorgvuldig vast in het elekt...
