Vacature details Solliciteren Functieomschrijving Als EVV'er (Eerst Verantwoordelijk Verzorgende) ben jij een vertrouwd gezicht voor bewoners en hun naasten. Jij zorgt voor duidelijkheid en continuïteit in de zorg. Op locatie Huys te Hoecke werk je in een kleinschalig en betrokken team, waar samenwerken en aandacht voor elkaar vanzelfsprekend zijn. Jouw rol Als EVV'er ben jij het vaste aanspreekpunt voor een aantal bewoners en hun familie. Je zorgt ervoor dat de zorg goed is afgestemd op de wensen en mogelijkheden van de bewoner. Je houdt overzicht, volgt afspraken en signaleert veranderingen in gezondheid of welzijn. Deze bespreek je met collega's en andere betrokkenen, zodat de zorg altijd passend blijft. Naast je coördinerende rol werk je mee in de dagelijkse zorg. Je ondersteunt bewoners bij de persoonlijke verzorging en bent zichtbaar op de afdeling. Collega's kunnen bij jou terecht met vragen en jij helpt mee om de zorg goed te organiseren. Met jouw aandacht, betrokkenheid en verantwoordelijkheidsgevoel draag je bij aan een veilige, warme en prettige leefomgeving.
