Als generalist ondersteun je inwoners op alle leefgebieden. Het uitgangspunt is dat het huishouden (waar nodig ondersteunt door het eigen netwerk) zoveel mogelijk zelf regie houdt op de uitvoering van het integrale ondersteuningsplan en het halen van de gestelde doelen. Jouw rol is ondersteunend en faciliterend aan dit proces. Je werkt dichtbij inwoners, hun netwerk en midden in de wijk. Je doet dat samen met collega's die - net als jij - een groot hart hebben voor de stad en voor de mensen die er wonen, werken en samenleven. Spreekt dit je aan? Dan komen we graag met je in contact. Jouw toegevoegde waarde als Generalist - Je ondersteunt Eindhovenaren bij het vinden van eigen oplossingen voor de uitdagingen waar zij tegenaan lopen, samen met de mensen die dichtbij hen staan. - Je werkt integraal, met aandacht voor de samenhang tussen werk, schulden, (mentale) gezondheid, wonen, opvoeding en veiligheid. Jij bent hun vaste contactpersoon. - Je zoekt altijd met de inwoner naar mogelijkheden binnen het eigen netwerk of in de sociale basis. Als dat niet toereikend is, biedt je zelf eerstelijns ondersteuning. - Je verzorgt de toegang tot aanvullende ondersteuning via de gemeente wanneer bijvoorbeeld Wmo‑begeleiding of budgetbeheer nodig is. - Je vervult de rol van casusregisseur. Bij complexe en of onveilige situaties wordt de regie mogelijk tijdelijk overgenomen. - Je signaleert veiligheidsrisico's, maakt zo nodig een veiligheidsplan, past de meldcode toe en werkt volgens de AVE‑afspraken samen met ketenpartners. - Je werkt nauw samen met samenwerkingspartners zoals gemeente, woningcorporaties, scholen, huisartsen en specialistische zorgaanbieders.
