Als gespecialiseerd longverpleegkundige bij Zorgbalans verricht je voornamelijk begeleidende en verpleegkundige werkzaamheden. Je informeert, instrueert en begeleidt cliënten met longaandoeningen en OSAS (slaapapneu) meestal in de thuissituatie en soms in één van onze locaties. Je geeft hen voorlichting over leefstijl, behandeling en sociale voorzieningen. Ook leg je saneringshuisbezoeken af bij cliënten met astma en/of allergieën. Uitgangspunt hierbij is dat onze zorgverlening de zelfredzaamheid en zelfmanagement van onze cliënten ondersteunt en daarbij maken we optimaal gebruik van technologie. Je werkt zelf volledig digitaal en zet de tablet in voor o.a. het comfort en monitoring van onze cliënten en mantelzorgers. Er is een nauwe samenwerking met het ziekenhuis, andere instellingen, paramedici en huisartsen. Tevens heb je een consult-en voorbeeldfunctie voor collega's en bevorder je deskundigheid op het gebied van longziekten binnen de organisatie. Wegens uitbreiding van ons team zoeken wij een nieuwe collega gespecialiseerd longverpleegkundige . Heb jij het profiel en de opleiding die we zoeken? - Je hebt een afgeronde HBO opleiding Verpleegkunde aangevuld met de aanvullende opleiding Longverpleegkundige of de bereidheid deze opleiding te gaan volgen - Je staat ingeschreven in het kwaliteitsregister - Daarnaast heb je werkervaring en affiniteit met cliënten met longziekten met een hoog complexe zorg vraag - Bij voorkeur beschik je over werkervaring in de wijkzorg - Je beschikt over sociale vaardigheden als tact, empathie en overtuigingskracht - Je bent zelfstandig, flexibel en hebt een reflecterend verm...
