We zijn op zoek naar een gespecialiseerd verzorgende (of EVV'er) voor 24 uur per week. Houd jij van aanpakken, ga je geen uitdaging uit de weg, en wil je de kwaliteit van zorg verbeteren? Dan zijn wij op zoek naar jou! Wat ga je doen? In de functie van Gespecialiseerd Verzorgende ligt de belangrijkste uitdaging in het goed leren kennen van de cliënten en het begrijpen van wat zij belangrijk vinden. Hierdoor kan waardevolle, persoonsgerichte zorg en verpleging worden gecoördineerd en verleend, zowel binnen de woonlocaties als bij de cliënt thuis. Het is van groot belang dat de regie zoveel mogelijk bij de cliënten zelf blijft. Als gespecialiseerd verzorgende (of EVV'er) wordt cliëntgerichte zorg geboden op basis van de afspraken in het zorgplan. Er wordt bijgedragen aan de continuïteit en kwaliteit van het welzijn van de cliënt, verpleegtechnische handelingen worden uitgevoerd, en eventuele bijzonderheden in de gezondheidstoestand worden gesignaleerd en besproken met de cliënt en/of hun familie. Daarnaast is de gespecialiseerd verzorgende (of EVV'er) het aanspreekpunt voor een aantal cliënten en hun contactpersonen. In overleg met hen wordt een individueel zorgplan geformuleerd dat aansluit op de wensen en behoeften van de cliënt, binnen de organisatorische kaders en de indicatie. De gespecialiseerd verzorgende (of EVV'er) is verantwoordelijk voor de coördinatie en evaluatie van de activiteiten uit het zorgplan en bewaakt de gemaakte afspraken met alle betrokkenen.
