Je stapt binnen op deze cruciale momenten. Je ondersteunt gezinnen bij het verminderen van stressfactoren en het versterken van het gezin. Zo krijgen zij weer meer grip op de situatie en regie over hun leven. Als jeugd- en gezinscoach kan je hierin écht het verschil maken voor gezinnen. Je werkt hierbij vanuit één van de 42 wijkteams van de gemeente Rotterdam. Wat jij gaat doen als jeugd- en gezinscoach Jij biedt vanuit het wijkteam basishulp, waarbij je verantwoordelijk bent voor je eigen caseload. Je legt huisbezoeken af en je brengt samen met het gezin in kaart wat er nodig is om de situatie te verbeteren. Dit doe je aan de hand van een hulpverleningsplan en je betrekt, indien mogelijk, het netwerk van het kind (bijvoorbeeld familieleden en vrienden) en het wijknetwerk (bijvoorbeeld scholen en CJG) hierin. Je biedt basishulp in de opvoeding. Daarnaast geef advies en ondersteuning op diverse leefgebieden, zoals zingeving, wonen, werk, financiën, sociale relaties en psychische gezondheid. Zie jij dat deze basishulp, die vanuit de wijkteams geboden wordt, onvoldoende is? Dan schakel je specialistische of intensievere/langdurige zorg in voor het gezin.
