Zoek je een afwisselende functie binnen kantoortijden, in een betrokken team in Amsterdam? Dan is werken bij Mentrum Trace iets voor jou. Als GGZ Verpleegkundige LVB bij Mentrum Trace begeleid en ondersteun je cliënten met een licht verstandelijke beperking en psychiatrische problematiek. Vanuit jouw verpleegkundige expertise sluit je aan bij het sociaal-emotionele ontwikkelingsniveau van de cliënt en lever je een belangrijke bijdrage aan de behandeling en begeleiding. Binnen het multidisciplinaire team ben jij een belangrijke schakel in de dagelijkse zorg. Je observeert, signaleert en stemt af met collega's, behandelaren en naasten om de best mogelijke zorg te bieden. 'Wij zijn een innovatief team dat graag de beste zorg wil leveren. Het is fijn om in zo'n team te werken ' Eva -Verpleegkundige Mentrum Trace- Wat ga je doen? - Coördineren van de zorg voor toegewezen cliënten; - Verstrekken en monitoren van medicatie; - Observeren van het mentale, emotionele en sociale functioneren van cliënten; - Onderhouden van contact met familie, naasten en ketenpartners; - Deelnemen aan multidisciplinaire overleggen; - Ondersteunen bij gesprekken tussen cliënt, arts en psychiater; - Bijdragen aan herstelgerichte psychiatrische zorg binnen de specialistische GGZ.
