Jij bent de schakel tussen vraag en antwoord, tussen behoefte en begeleiding. Dankzij jouw inzet kunnen gezinnen vanuit hun eigen kracht problemen oplossen, met jouw ondersteuning waar dat nodig is. Je kijkt naar gezinnen met de overtuiging dat zij zelf regie kunnen voeren over hun situatie. Met die visie ondersteun je hen bij het vinden van eigen oplossingen en breng je in kaart welke aanvullende hulp nodig is. Jij zorgt dat die hulp op het juiste moment beschikbaar komt. Als jeugdprofessional werk je zowel vanuit de gemeentelijke toegang als een dagdeel per week in een huisartsenpraktijk. Het uitgangspunt in je werk is dat kinderen en gezinnen zoveel mogelijk vanuit eigen kracht problemen oplossen en hierbij ondersteun je hen waar nodig. Je brengt de hulpvraag van jeugdigen en hun gezinnen zorgvuldig in kaart en schakelt passende ondersteuning in. Bij complexe casussen neem jij de regie, zodat alle betrokken hulp goed op elkaar blijft afgestemd. Hierbij heb je altijd aandacht voor de veiligheid van het kind. Jij - analyseert hulpvragen van kinderen, ouders en andere betrokkenen, met voortdurende aandacht voor veiligheid. - geeft advies, consultatie en informatie aan gezinnen én professionals zoals scholen, zorgaanbieders en huisartsen. - biedt kortdurende ondersteuning (gemiddeld vijf gesprekken) aan kinder...
