Zorgcentrum het Bildt zoekt per direct een Helpende plus voor in de Thuiszorg Kun jij niet wachten om het beste uit jezelf en een ander te halen en je talenten in te zetten? Dan is de afdeling Thuiszorg van Zorgcentrum het Bildt op zoek naar jou! Wat ga je doen? Met een fijne club collega's proberen we het verschil te maken in het leven van onze cliënten. Jij kunt onderdeel uitmaken van dit deskundige team. Jij, als helpende plus, speelt hierin een belangrijke rol. Je geeft de cliënten persoonlijke aandacht met de zorg en begeleiding die zij nodig hebben. Binnen de thuiszorg van Zorgcentrum het Bildt bieden we wijkverpleging aan op basis van de Zorgverzekeringswet (Zvw). Vanuit deze indicatie ondersteun je mensen thuis, zodat zij zo zelfstandig mogelijk kunnen blijven functioneren. Naast deze reguliere thuiszorg bieden wij ook het Volledig Pakket Thuis (VPT) aan voor cliënten met een Wlz-indicatie. Als Helpende plus binnen dit team ga je je werkzaamheden vorm geven. Het team bestaat uit een mix van collega's met diverse opleidingsniveaus. Cliënten die zorg ontvangen van het VPT team hebben een indicatie vanuit de Wet Langdurige Zorg. De cliënt blijft zelfstandig wonen, in het eigen huis, met alle zorg en ondersteuning die daarvoor nodig is. Zo kan de cliënt het leven voortzetten, op zijn/haar eigen manier. Naast zorg, huishoudelijke ondersteuning en de dagelijkse maaltijden wordt bijvoorbeeld ook de dagbesteding naar huis gehaald. Spelletjes spelen, een visje halen of even een blokje om, het is maar net wat de cliënt nodig heeft.
