Solliciteer en versterk ons team Volledig Pakket Thuis in Opsterland! Wat ga je doen in het team Volledig Pakket Thuis? Het team Volledig Pakket Thuis zorgt voor mensen die een WLZ-indicatie hebben in de thuissituatie. Veel van de cliënten hebben een vorm van dementie of om een andere redenen behoefte aan structuur in de dag en ondersteuning bij eigen regie en autonomie. Omdat je werkt binnen een VPT-team met WLZ-cliënten, is er meer tijd en aandacht voor persoonlijke begeleiding en het stimuleren van zelfredzaamheid. Als helpende plus/cliëntondersteuner ga jij ondersteunende werkzaamheden uitvoeren bij de cliënt thuis. Denk hierbij aan het begeleiden, stimuleren, ondersteunen bij en/of overnemen van lichamelijk verzorgende taken zoals het wassen, aankleden, aantrekken van steunkousen, het aanreiken van medicatie en het toezien op inname. Daarnaast onderneem je samen met de cliënt welzijnsactiviteiten die aansluiten bij de behoefte en interesse van de cliënt, waaronder bijvoorbeeld: de krant lezen, spelletjes, wandelen, uitjes, boodschappen doen. De zorg verricht je volgens het zorgplan en op aanwijzing van de cliëntcoach. Je verricht ook huishoudelijke werkzaamheden, waaronder schoonmaken, de was en eventueel bakken/koken. Je stimuleert de zelfredzaamheid van de cliënt en neemt alleen over waar nodig. Je signaleert veranderingen bij de cliënt, rapporteert hierover en stemt af met de cliëntcoach.
