Kom dan ons Thuiszorgteam versterken. Want daar zijn we hard op zoek naar een: Helpende Plus in de Thuiszorg (avonddiensten) Aantal uur per week bespreekbaar Dit ga je straks met veel plezier doen. In het kort. - Bied persoonlijke zorg en individuele begeleiding aan cliënten in de thuiszorg, afgestemd op hun zorgvraag en met aandacht voor hun lichamelijke en mentale welzijn. - Ondersteun cliënten bij de dagelijkse verzorging en voer aanvullende zorgtaken uit, zoals medicatie aanreiken, bloedsuikers meten, insuline toedienen en basiswondzorg. - Signaleer veranderingen in de gezondheid van cliënten en rapporteer deze zorgvuldig. - Werk binnen het Langer Vitaal Thuis-project aan het bevorderen van de zelfredzaamheid en stimuleer cliënten om zoveel mogelijk de eigen regie over hun gezondheid en welzijn te behouden. - Werk vanuit positieve gezondheid, denk vooruit en draag met jouw ideeën bij aan het verbeteren van de zorg. - Blijf jezelf ontwikkelen. Met jouw leergierige houding en de wil om te groeien ben je bij ons op je plek. - Werk in de avonduren van 18.00 tot 21.00 uur en maak samen met je collega's het verschil voor onze cliënten. Hier ga je werken. Wij zijn Samen, dé partner bij het ouder worden in de gemeente Schagen en Hollands Kroon. Cliënten kunnen bij ons terecht voor thuiszorg, wonen met zorg, revalidatie & behandeling, welzijnsdiensten en dagactiviteiten. Jij gaat werken in regio Schagen. Er zijn drie thuiszorgteams die met aandacht en tijd zorg leveren bij cliënten thuis. Je wordt onderdeel van een vast en betrokken team. Als het nodig is, spring je ook bij in een ander team.
