Laat jij de cliënt het zoveel mogelijk zelf doen, maar ben je er als hij of zij je nodig heeft? Wat ga je doen? Als Helpende Plus werk je bij De Nieuwe Munt in Tegelen met cliënten die tijdelijk verblijven om te revalideren. Iedere cliënt heeft een eigen hersteldoel en werkt daar in een passend tempo naartoe. Waar de één ondersteuning nodig heeft bij de persoonlijke verzorging, heeft een ander vooral behoefte aan aanmoediging en vertrouwen. Tijdens de dagelijkse zorg kijk je steeds naar wat iemand zelf kan. Je stimuleert cliënten om actief mee te werken aan hun herstel en ondersteunt wanneer dat nodig is. Daarnaast voer je de zorgtaken uit die bij jouw functie horen, waaronder medicatiezorg. Goed observeren is een belangrijk onderdeel van je werk. Je merkt veranderingen in de lichamelijke of mentale toestand van een cliënt op, bespreekt bijzonderheden met je collega's en legt deze zorgvuldig vast in het cliëntdossier. Je werkt nauw samen met verzorgenden, verpleegkundigen, behandelaren en andere betrokken disciplines. Samen zorgen jullie voor een veilige revalidatieperiode en bereiden jullie de cliënt voor op een zo zelfstandig mogelijke terugkeer naar huis of een andere passende woonomgeving.
