Hier ondersteun je cliënten met een volledig pakket aan zorg en welzijn, zodat zij zelfstandig en waardig in hun eigen omgeving kunnen blijven wonen. Je combineert persoonlijke verzorging, begeleiding en welzijnswerk zodat cliënten in hun vertrouwde omgeving kunnen blijven wonen. Je kijkt samen met cliënten naar wat zij nog zelf kunnen, waarbij zelfredzaamheid en persoonlijke aandacht centraal staan. In deze functie kijk je echt naar wat de cliënt nodig heeft en dit kan ook af en toe zijn het spelen van een spelletje of het maken van een wandeling.
