Huisarts in loondienst Geschreven door Gezondheidscentrum Merenwijk op 1 september 2026 . Wij zijn op zoek naar een enthousiaste Huisarts in loondienst die ons team komt versterken voor circa 27 uur per week (3 werkdagen) . Ben jij een betrokken huisarts die graag werkt in een hecht team én wil bijdragen aan de ontwikkeling van de ketenzorgprogramma's? Dan maken wij graag kennis met je! Wie zijn wij? Gezondheidscentrum Merenwijk is een grote, levendige huisartsenpraktijk in Leiden. Wij hebben een diverse patiëntenpopulatie van ongeveer 8000 patiënten. Ons team bestaat uit: - 3 duopraktijken en 2 vaste ondersteunende huisartsen - 3 praktijkondersteuners somatiek (POH-S) - 2 praktijkondersteuners GGZ - Een praktijkmanager en financieel medewerker - Een betrokken team van assistentes, baliemedewerkers en geneeskundestudenten Wij werken nauw samen met andere disciplines binnen het gezondheidscentrum, zoals psychologie, fysiotherapie, diëtetiek en het diagnostisch laboratorium. De sfeer? Open, laagdrempelig en collegiaal . We lunchen samen, drinken samen koffie en hebben tweemaal per dag een kort overlegmoment om de dag door te nemen. Er is veel aandacht voor elkaar én voor jouw professionele ontwikkeling. Wat ga je doen? Als huisarts in loondienst realiseer je hoogwaardige en afgestemde eerstelijnszorg door het uitvoeren van medisch-generalistische huisartsgeneeskundige zorg. Daarnaast vervul je een regierol binnen de zorgketenprogramma's en draag je bij aan de chronische patiëntenzorg binnen onze praktijk. Je werkzaamheden bestaan onder andere uit: - Het voeren van spreekuren en het afleggen van visites - Het diagnosticeren, behandelen en begeleiden van patiënten met uiteenlopende zorgvragen - Het coördineren en afstemmen van zorg met andere zorgverleners binnen en buiten het gezondheidscentrum - Het ondersteunen van de POH-S en verder ontwikkelen van de chronische patiëntenzorg, waaronder de ketenzorgprogramma's: diabeteszorg, ...
