We zoeken een collega huisarts voor 3 dagen per week (in loondienst). Je bent verantwoordelijk voor een aantal programma's en samenwerkingsverbanden vanuit HCWB en gesprekspartner in de regio. Ook verbindt je huisartsen, ziekenhuis, VVT, gemeenten en andere regionale partners en vertaalt gezamenlijke ambities naar concrete programma's, afspraken en resultaten. Vanuit deze rol lever je onder meer een bijdrage aan de uitvoering van de IZA/AZWA-regioplannen, de beweging naar passende zorg en de ontwikkeling van hechte wijkverbanden en het regionale eerstelijnssamenwerkingsverband (RESV). Je verbindt strategie, zorginhoud en uitvoering en zorgt voor samenhang, resultaat en een goede vertaling naar de aangesloten huisartsenpraktijken.
