IMEAN is een groeiende organisatie gespecialiseerd in cultuursensitieve zorg voor ouderen, met bijzondere aandacht voor mensen met een migratieachtergrond én mensen met (beginnende) dementie. Wij helpen cliënten en hun families op een manier die aansluit bij hun taal, cultuur, gewoonten en identiteit. Wat ga je doen? Als casemanager dementie begeleid je cliënten met geheugenproblemen of een dementiediagnose én hun mantelzorgers vanaf het moment van signalering tot en met de verdere fasen van het ziekteproces. Jij vormt de verbindende schakel tussen cliënt, familie, huisarts, wijkverpleging, dagbesteding en andere betrokken zorgverleners. Je bent voor veel families hét vertrouwde aanspreekpunt, mede omdat jij hun taal spreekt en hun culturele context begrijpt. Jouw werkzaamheden omvatten onder andere: - uitvoeren van (vroeg)signalering en intakegesprekken bij cliënten met geheugenproblemen - begeleiden en coachen van mantelzorgers en familieleden - opstellen, uitvoeren en evalueren van zorg- en ondersteuningsplannen - coördineren van de zorg rondom de cliënt (ketenregie) - intensief samenwerken met huisartsen, specialisten, wijkteams, dagbesteding en welzijnsorganisaties - tijdig herkennen en bespreken van veranderingen in het ziektebeeld - voorlichting geven over dementie, zorgopties en cultuursensitieve ondersteuning - actieve bijdrage leveren aan de verdere ontwikkeling van het casemanagement binnen IMEAN Wie ben jij?
