The posting

Als hulpverlener woonbegeleiding bied je intensieve partiële of integrale ambulante hulp- en dienstverlening aan (jong)volwassenen met een woonproblematiek. Je werkt zowel op huisbezoek als voor het cliënteel in de residentiële opvang in het werkingsgebied Meetjesland Schelde-Leie.

Vanuit de kracht van de cliënt werk je aan het vergroten van de competenties om zelfstandig te wonen. Je verkent de problematiek door middel van huisbezoeken en stelt een begeleidingsplan op, samen met de cliënt. In functie van de behoeften van de cliënt bepaal je aan welke levensgebieden (sociaal-materiaal, relationeel, psychische en fysische gezondheid) jullie werken en hoe de hulpverlening het beste wordt georganiseerd. Met je cliënt loop je samen een (kortdurige) begeleiding en waak je over een nauwkeurige administratieve opvolging per dossier. Hierbij werk je in nauwe samenwerking met zowel je collega’s alsook externe partners om de best mogelijke hulpverlening aan te bieden.

Je werkt één avond per week (max. tot 19u) om ook cliënten te kunnen ontvangen die zich overdag moeilijker kunnen vrijmaken.

- Je hebt een diploma hoger onderwijs (bachelor of graduaat) in een menswetenschappelijke richting (zoals toegepaste psychologie, orthopedagogie of sociaal werk) - Je bent vertrouwd met psychosociale begeleiding en hebt bij voorkeur affiniteit met de thuislozenproblematiek. - Je werkt graag zelfstandig en weet het eigen werk goed te plannen en te organiseren. - Je speelt vlot in op nieuwe of wijzigende omstandigheden en wijzigt je aanpak wanneer nodig. - Je hebt oog voor een vlotte en nauwkeurige administratieve opvolging. - Je bent in het bezit van een rijbewijs B en kan je makkelijk verplaatsen in het werkingsgebied Meetjesland Schelde-Leie.
