Dankzij jouw overzicht en snelle afstemming krijgen de verwijzer en de cliënt duidelijkheid en komt die op een plek die echt past in de regio Rotterdam / Woerden - Sociaal - Liefdevol - Aandacht - Jouw werk - Jouw kwaliteiten en motivatie - Wat biedt Ontmoeting? - Waar ga je werken? Jouw werk Vanaf de eerste aanmelding houd jij de lijn vast. Je luistert goed, stelt de juiste vragen en brengt samen met collega's in beeld welke ondersteuning nodig is. Zo zorg je voor een vlot verloop van de aanmelding en passende ondersteuning en weet de cliënt waar deze aan toe is. - Je beoordeelt dossiers en voert intakes, live of online. Zo krijg je snel een compleet beeld van de cliënt en de hulpvraag. - Je regelt het proces van aanmelding tot plaatsing en houdt het ECD, de mailbox en wachtlijsten bij. Hierdoor blijft het traject duidelijk en blijft er voortgang in het traject van de cliënt. - Je overlegt met collega's en locaties over plaatsing, doorstroom en uitstroom gericht op plaatsingen vanuit de FZ en Wlz. Samen zorg je voor overzicht en tijdige instroom. - Je bezoekt verwijzers en netwerkpartners en de kerngroepen FZ en Wlz. Je deelt online en offline het verhaal van de impact van begeleiding vanuit de FZ en Wlz. Zo bouw je aan betere interne en externe samenwerking en meer passende mogelijkheden voor toekomstige cliënten. Ontmoeting blijft hierdoor in beeld bij verwijzers. Ons team is verantwoordelijk voor het hele werkgebied van Ontmoeting, van regio Rotterdam / Woerden tot de Veluwe en Noord-Holland.
