The posting

  • Je begeleidt een aantal cliënten met meervoudige (GGZ) problematieken individueel aan huis
  • Je stelt samen met de cliënt en zijn omgeving een begeleidingsplan op met hersteldoelen; we verwijzen hier naar de specifieke projectfiche van de werking rond jongvolwassenen (bijlage)
  • Je tracht samen met de actoren rond de cliënt een vertrouwensband op te bouwen om zo de nodige stappen te zetten in zijn/haar herstelproces en maatschappelijke integratie. Veelvuldige huisbezoeken zijn hierbij onontbeerlijk
  • Met de zorgbehoeften van de individuele zorggebruiker als uitgangspunt, is de herstel-ondersteunende begeleiding gericht op verschillende levensdomeinen
  • Diplomavereisten: Bachelor in de menswetenschappen, psychiatrisch of sociaal verpleegkundigen, ergotherapie, orthopedagogie, sociaal assistent…
  • Je beschikt over een rijbewijs en eigen wagen
  • Je hebt interesse in psychiatrische zorg en werkt op een respectvolle, mensgerichte manier.
  • Je bent een teamspeler die ook zelfstandig kan werken en initiatief neemt.
  • Je bent open, communicatief en betrokken, met een constructieve houding.
  • Je behoudt het overzicht in stressvolle situaties en werkt oplossingsgericht.
  • Ervaring in de geestelijke gezondheidszorg is een pluspunt, maar geen vereiste.
