Binnen Partners voor Jeugd werken we daar iedere dag aan. Voor William Schrikker Gezinsvormen zoeken wij een pleegzorgwerker/ambulant hulpverlener voor regio Midden-West, met als standplaats Leiden. In deze combinatiefunctie begeleid je kinderen, jongeren en gezinnen in complexe en kwetsbare situaties. Je werkt zowel binnen de pleegzorg als in de ambulante hulpverlening. Hierdoor is je werk afwisselend en kun je vanuit verschillende vormen van ondersteuning bijdragen aan een veilige en stabiele ontwikkeling van het kind. De functie Als pleegzorgwerker begeleid je kinderen die tijdelijk of langdurig niet thuis kunnen wonen en in een pleeggezin verblijven. Je ondersteunt het kind, de pleegouders en de ouders en zorgt ervoor dat de plaatsing zo goed mogelijk verloopt. Binnen het ambulante deel van de functie begeleid je kinderen, jongeren en ouders in hun eigen leefomgeving. Samen met het gezin werk je aan veiligheid, opvoedvragen en het versterken van de zelfredzaamheid. Je sluit aan bij wat het gezin nodig heeft en betrekt het netwerk waar mogelijk. Je voert huisbezoeken uit, volgt de ontwikkeling van het kind en begeleidt waar nodig contactmomenten tussen ouders en kinderen. Je houdt zicht op veiligheid, stabiliteit en continuïteit en signaleert tijdig wanneer bijsturing nodig is. Je hebt de regie over het hulpverleningstraject, stelt begeleidingsplannen op en stemt af met scholen, gemeenten en andere betrokken professionals. In complexe situaties weet je snel te schakelen en passende beslissingen te nemen. Je werkt zelfstandig, maar bent onderdeel van een betrokken en deskundig team in regio Midden-West.
