Dit wordt jouw functie Kinderen op een stabiele, veilige en verantwoorde manier opvoeden is helaas niet in elk gezin vanzelfsprekend. Bijvoorbeeld door financiële problemen, gebrek aan kennis of lastig gedrag van de kinderen. Gezinnen kunnen dan bij het wijkteam terecht om hulp te krijgen bij onder andere de opvoeding en het opgroeien van hun kinderen. Als jeugdconsulent ben je gezinsbegeleider: jij bent het eerste aanspreekpunt voor gezinnen. Je draagt als hulpverlenende coördinator bij aan het mogelijk maken van een veilige omgeving voor het kind. Je staat hierbij aan de zijlijn: de inspanning moet uit het gezin zelf komen. Als jeugdconsulent is elke werkdag afwisselend en uitdagend. Het belangrijkste deel van je werk bestaat uit het analyseren van hulpvragen en de eigen kracht van cliëntsystemen. Je legt huisbezoeken af, en onderhoudt contact met onder andere hulpverleners, scholen en andere ketenpartners. Gezinsdoelen en ontwikkelingen rapporteer je zorgvuldig. Als jeugdconsulent werk je veelal zelfstandig; je bent verantwoordelijk voor een aantal gezinnen (eigen caseload) en plant je eigen afspraken in. Belangrijke beslissingen over gezinnen bespreek je in multidisciplinair overleg. De werksfeer binnen de Sociale Wijkteams is informeel. Het is hard werken, maar je kunt van je collega’s veel steun verwachten. Zowel op inhoudelijk als op persoonlijk vlak. Als Jeugdconsulent ga je

- Op huisbezoek, je komt dus bij gezinnen thuis;

- Gesprekken voeren met het gezin om de hulpvraag in kaart te brengen. Dit kan van alles zijn, denk aan vragen over opvoeding, financiën en veiligheid;

- Als de hulpvraag duidelijk is, een plan van aanpak opstellen om de gezinsdoelen te behalen;

- Specialistische hulpverlening in gang zetten wanneer dit nodig is;

- Contact onderhouden met de hulpverleners om te monitoren hoe het met de doelen gaat;

- Rapporteren: je legt doelen van de cliënten duidelijk vast in rapportages;

- Overleggen met collega’s over j
