Daarom doe je wat je doet. Je gaat in gesprek met gezaghebbende ouders en zorgaanbieders, zodat de hulpvraag glashelder wordt. Doordat je stevig in je schoenen staat, de Jeugdwet kent en SKJ geregistreerd bent, leid jij diverse hulpvragers toe naar de juiste zorg volgens het principe 1 klant, 1 plan, 1 regisseur . Wat zijn je taken? - Het afleggen van huisbezoeken en keukentafelgesprekken; - Advies geven aan jeugdigen/opvoeders zodat zij waar nodig zelf uit de voeten kunnen; - Bepalen welk gezin welke ondersteuning nodig heeft en dit organiseren; - Signalen herkennen en reageren op vermoedens van kindermishandeling. Functie-eisen Je bent enthousiast en hebt een gezonde dosis mensenkennis. Daarnaast heb je: - Een afgeronde Hbo of Wo opleiding; - SKJ-registratie; - Kennis van de Jeugdwet en aanverwante wet- en regelgeving; - Een aantal relevante jaar werkervaring, denk aan jeugdconsulent, gezinscoach of jeugdbeschermer.
