De gemeente biedt ondersteuning bij de opvoeding, begeleiding en zorg voor kinderen. Soms kunnen gezinnen dit zelf of met hulp van familie, vrienden of buren, maar in andere gevallen is extra hulp nodig en vraagt een gezin hulp vanuit de Jeugdwet bij de gemeente aan. Als Jeugdconsulent help je gezinnen woonachtig in Wassenaar met uiteenlopende hulpvragen. Je maakt samen met het gezin een analyse van de hulpvraag en verwerkt dit in een ondersteuningsplan. Je geeft advies over de Jeugdwet, o.a. PGB en Maatwerk, en werkt hierin samen met ketenpartners en zorgaanbieders. Waar nodig zet je gespecialiseerde hulp in en maak je dit administratief in orde zodat de zorgaanbieder kan starten. Je monitort samen met ouders de ingezette hulp en waar nodig stel je de doelen bij of sluit je de ingezette hulp af. De Jeugdconsulent is verantwoordelijk voor casusregie bij meervoudige en complexe problematiek en veiligheidszaken. De Jeugdconsulent voert casusregie zo kort als mogelijk en zo lang als nodig. Als Jeugdconsulent is geen dag hetzelfde, het is een uitdagende en afwisselende functie waarin je veel verantwoordelijkheid krijgt. Je komt te werken in een enthousiast en behulpzaam team waar ruimte is om van elkaar te leren en te ontwikkelen. Taken en verantwoordelijkheden: - Verrichten van onderzoek ter verduidelijking van de hulpvraag van jeugdigen/gezinnen n.a.v. (her)aanvraag jeugdhulp; - Bevorderen van eigen kracht en mogelijkheden van het gezin en het inzetten van het netwerk rondom het gezin...
