Als jeugdconsulent bij Gemeente Den Helder kijk je verder dan het dossier. Je stelt de juiste vragen, brengt gezinssituaties zorgvuldig in kaart en onderzoekt welke ondersteuning nodig is. Met jouw analytische blik en daadkracht breng je vastgelopen dossiers weer in beweging en voer je regie op hulpverleningstrajecten. Zo help je jeugdigen en gezinnen écht verder, waarbij het belang van het kind altijd centraal staat. Wat je gaat doen Binnen Team Jeugd van Gemeente Den Helder werk je aan complexe bestaande jeugdzaken. De focus ligt op het terugdringen van de wachtlijst en het realiseren van voortgang in dossiers die specialistische aandacht vragen. Jouw werkzaamheden bestaan uit: - Het analyseren van complexe jeugdzaken en het beoordelen van de ondersteuningsbehoefte van jeugdigen en gezinnen. - Het opstellen van ondersteuningsplannen en adviseren over passende jeugdhulp. - Het voeren van procesregie op lopende hulpverleningstrajecten en bewaken van de gemaakte afspraken. - Het signaleren van veiligheidsrisico's en opstellen van veiligheidsplannen, bijvoorbeeld bij complexe gezinssituaties en (v)echtscheidingen. - Het afstemmen met ouders, zorgaanbieders en andere ketenpartners om dossiers verder te brengen en passende ondersteuning te organiseren. Je wordt specifiek ingezet op complexe bestaande dossiers. Je draait geen crisisdiensten en voert geen nieuwe intakes uit. Hierdoor kun jij je richten op inhoudelijke verdieping, zorgvuldige besluitvorming en concrete resultaten.
