Jeugdconsulent Zorg en Onderwijs Rijswijk (ZH) Over de functie ft 33137 Je bent de schakel tussen onderwijs en jeugdhulp en werkt nauw samen met scholen, ouders en ketenpartners. Je signaleert hulpvragen vroegtijdig en helpt deze te vertalen naar passende ondersteuning. Daarbij kijk je steeds naar het kind én de context en stem je zorg en onderwijs goed op elkaar af. Je werkt outreachend, denkt mee op casus- en beleidsniveau en draagt bij aan de ontwikkeling van de samenwerking in de regio. Wat ga je doen - Je signaleert zorgvragen binnen het onderwijs en helpt deze te duiden en te normaliseren. - Je bent het vaste aanspreekpunt voor scholen en het netwerk. - Als er sprake is van een indicatie voor de Jeugdwet, neem je aanvragen voor jeugdhulp in behandeling en schakel je passende hulp in. - Je doet onderzoek met de GIZ en schrijft een ondersteuningsplan. - Je denkt mee met scholen en ketenpartners, deelt kennis en ondersteunt bij het verbeteren van processen. Eisen - Vloeiende beheersing van de Nederlandse taal op C1-niveau. - Je hebt minimaal 2 jaar aantoonbare werkervaring met het uitvoeren van de Jeugdwet, en je benoemt duidelijk waar dit is opgedaan. - Je hebt een aantoonbaar afgeronde opleiding op minimaal hbo-bachelor niveau. - Je bent aantoonbaar SKJ- of BIG-geregistreerd en benoemt in het cv het registratienummer. - Je hebt aantoonbare werkervaring met systeemgericht werken. Wensen - Je hebt minimaal 3 jaar aantoonbare werkervaring met het toepassen van de GIZ-methodiek en het werken volgens de meldcode en het afwegingskader passend onderwijs en jeugdhulp, duidelijk weergegeven in het cv. - Je hebt aantoonbare werkervaring met het zelfstandig behandelen van jeugdcasuïstiek binnen een gemeentelijke organisatie. - Je hebt aantoonbare werkervaring met regievoering en samenwerking in een (lokaal) zorgnetwerk met tenminste een onderwijs stakeholder, duidelijk weergegeven in het cv. - Je hebt aantoonbare werkervaring met reg...
