Voorschoten, Zuid-Holland Begeleider jeugdconsulenten - procesregisseur Functie omschrijving Kernverantwoordelijkheden Je helpt de jeugdconsulenten bij: - Het voeren van procesregie over casussen waarin Jeugdhulp betrokken is. - Het analyseren en beoordelen van hulp- en zorgvragen met betrekking tot opvoeden en opgroeien. - Het zoeken naar optimale aansluiting bij de hulpvraag/hulpbehoefte van het cliëntsysteem, met oog voor de eigen kracht van ouders en jeugdigen. - Het neerzetten van kaders waarbinnen de hulpverlening zijn werk uitvoert. Als procesregisseur: - Voer je procesregie in complexe en multiprobleemzaken op het gebied van jeugd en volwassenen. - Werk je met doelgroepen die niet (volledig) zelfredzaam zijn, zorgwekkende zorgmijders of inwoners met meervoudige problemen op gebieden als wonen, zorg, werk, school of justitie & politie. Team en context Team Sociaal Service Team: Het team bestaat uit consulenten, administratieve en kwaliteitsmedewerkers en is verantwoordelijk voor uitvoering van de Jeugdwet en de Wmo, inclusief indicatie en toewijzing van passende hulp en coördinatie van opvang van ontheemden met aandacht voor integratie. Organisatie en werkwijze - De werkzaamheden zijn ingebed in de organisatie en worden in teamverband uitgevoerd. - De functie neemt deel aan reguliere werkoverleggen en legt verantwoording af aan een opdrachtgever of leidinggevende. - De werkvorm is bepaald in onder andere locatie en planning voor de te leveren resultaten. Functie eisen (1) en -wensen (•) Eisen: - Aantoonbare afgeronde opleiding op minimaal hbo-bachelor niveau - Minimaal 4 jaar aantoonbare werkervaring in de afgelopen 7 jaar als jeugdconsulent, jeugd- en gezinsprofessional en/of Ouder- en Kindadviseur binnen het jeugddomein - Aantoonbare werkervaring in de afgelopen 2 jaar binnen de uitvoering van de Jeugdwet, waarbij ervaring is opgedaan met het onderzoeken van hulpvragen, indiceren en/of beschikken van jeugdhulp - Beschi...
