Begeleider jeugdconsulenten – procesregisseur Voorschoten Je ondersteunt en coacht minder ervaren jeugdconsulenten en brengt jouw kennis en ervaring in bij complexe casussen. Daarnaast vervul je een belangrijke rol als procesregisseur binnen het sociaal domein. Je werkt aan vraagstukken rondom jeugd, volwassenen en multiproblematiek, waarbij je steeds de juiste afwegingen maakt en richting geeft aan passende ondersteuning. Je draagt bij aan een professionele, betrokken en klantgerichte uitvoering van de werkzaamheden. In deze opdracht krijg je ruimte om zelfstandig te werken en tegelijk nauw samen te bewegen met collega's en betrokken partijen. Wat ga je doen - Je ondersteunt en coacht minder ervaren jeugdconsulenten. - Je draagt kennis en kunde over bij complexe gevallen. - Je voert procesregie over casussen waarin Jeugdhulp zit. - Je analyseert en beoordeelt hulp- en zorgvragen met betrekking tot opvoeden en opgroeien. - Je zoekt optimaal aansluiting bij de hulpvraag c.q. hulpbehoefte van het cliëntsysteem met inachtneming van de eigen kracht van de ouders en de jeugdigen. - Je zet kaders neer waarbinnen de hulpverlening zijn werk gaat doen. - Je voert de procesregie in complexe en multiproblem zaken op het gebied van jeugd en volwassenen. - Je werkt met inwoners die niet zelfredzaam zijn, zorgwekkende zorgmijders zijn of problemen hebben op het gebied van wonen, zorg, werk, school of justitie & politie. Eisen - Je hebt een aantoonbare afgeronde opleiding op minimaal hbo-bachelor niveau. - Je hebt minimaal 4 jaar aantoonbare werkervaring in de afgelopen 7 jaar als jeugdconsulent, jeugd- en gezinsprofessional en/of Ouder- en Kindadviseur binnen het jeugddomein. - Je hebt aantoonbare werkervaring in de afgelopen 2 jaar binnen de uitvoering van de Jeugdwet, waarbij ervaring is opgedaan met het onderzoeken van hulpvragen, indiceren en/of beschikken van jeugdhulp. - Je bent beschikbaar per 17 november 2026 voor gemi...
