Brummen, Gelderland Jeugdonsulent Functie omschrijving Functie Wij zoeken twee ervaren jeugdconsulenten die zelfstandig casussen vlot kunnen afhandelen. Voorkeur : 36 uur per week. Functieomschrijving - Voeren van gesprekken met inwoner, ouders en jongere om de vraag helder te krijgen. - Stimuleren en faciliteren van ouders en jongere om regie (te) nemen en te houden. - Verkennen van mogelijkheden van het gezin, het persoonlijke netwerk en de sociale omgeving. - Opstellen van een plan van aanpak met wenselijk resultaat en tijdpad. - Inzetten van expertise om zo vroeg mogelijk de juiste ondersteuning te bieden. - Coördineren van ingezette ondersteuning en tussentijds bijsturen waar nodig. - Optreden als specialist op het jeugdhulpaanbod en contact onderhouden met zorgaanbieders. - Vanuit de dagelijkse praktijk input leveren aan beleidsadviseurs over kwalitatief goede en efficiënte voorzieningen in de jeugdhulp. - Samenwerken met collega's om tot de juiste aanpak te komen en de regie te houden waar nodig. - Zorgdragen voor correcte vastlegging en het geven van opdrachten aan organisaties voor maatwerkvoorzieningen. - Werken onder leiding en toezicht van de Coördinator Team voor Elkaar. Wij vragen - Ervaring als jeugdconsulent en het vermogen om casussen zelfstandig en efficiënt af te handelen. - Stressbestendigheid en het vermogen om met complexe vraagstukken om te gaan. - Goede gespreksvaardigheden en het vermogen ouders en jongeren te motiveren en te faciliteren. - Kennis van en ervaring met coördinatie van jeugdhulp en samenwerking met zorgaanbieders. - Pragmatische instelling: 1 gezin, 1 plan, 1 aanpak. Functie eisen (1) en -wensen (•) Eisen: - Aantoonbare afgeronde opleiding op minimaal hbo-bachelor niveau - De kandidaat beschikt over een geldige SKJ-registratie.
