Als jeugdprofessional intensieve hulpverlening stap jij in wanneer het écht complex wordt. Je werkt intensief met gezinnen in complexe situaties waar meerdere problemen tegelijk spelen en waar veiligheid, ontwikkeling en stabiliteit onder druk staan. Je bent betrokken, vasthoudend en weet wanneer je moet opschalen of juist afschalen. Je werkt in de leefwereld van gezinnen: thuis, in de wijk en samen met partners. - Voeren van intensieve gesprekken en twee à drie keer per week op huisbezoek gaan - In kaart brengen van risico's, veiligheid en dynamieken - Opstellen van een plan samen met het gezin en diens netwerk - Bieden van intensieve begeleiding en opvoedondersteuning - Interveniëren in crisissituaties en actie ondernemen waar nodig - Voeren van regie en afstemmen met betrokken professionals en instanties - Continu bewaken van de voortgang en veiligheid Je werkt in wijkteams, op scholen of bij huisartsen in de regio Maassluis, Vlaardingen en Schiedam, altijd midden in de praktijk. En dat doe je niet alleen: je maakt deel uit van een betrokken team van collega's en gedragswetenschappers, waar je op kunt terugvallen, kunt sparren en samen leert. Via het Leerhuis wordt continu geïnvesteerd in jouw ontwikkeling en vakmanschap. Samen wordt gebouwd aan sterke, toekomstbestendige jeugdhulp: dichtbij, mensgericht en gericht op wat wél kan. Jij speelt daarin een sleutelrol. - HBO-opleiding, bijvoorbeeld Social Work of Pedagogiek - Ervaring in het werken met gezinnen in complexe situaties waarbij de veiligheid in het geding is - Kennis van de veiligheidsketen - SKJ-registratie (vereist) - Toegankelijk, snel schakelend en in staat om vertrouwen op te bouwen bij zowel gezinnen als zorgprofessionals - Salaris CAO Sociaal Werk, schaal 9, tussen € 3....
