Je adviseert cliënten en naasten over passende zorg, beoordeelt zorgvragen en brengt cliënten, verwijzers en zorgprofessionals bij elkaar. Wat ga je doen Als Klantadviseur ben je het aanspreekpunt voor nieuwe cliënten, hun naasten en verwijzers. Vaak komen zij bij jou op een moment waarop er veel geregeld moet worden. Jij luistert naar hun vraag, geeft duidelijke informatie en advies over de mogelijkheden binnen Amaris en beoordeelt of de zorgvraag daarbij aansluit. Daarbij combineer je het belang van de cliënt met een goede inzet van de beschikbare zorgcapaciteit. Je kent de zorg, kunt verschillende belangen afwegen en weet welke stappen nodig zijn om tot een passende oplossing te komen. - adviseert cliënten en naasten over passende zorg en ondersteuning binnen Amaris; - beoordeelt zorgvragen en brengt samen met de cliënt en betrokken zorgprofessionals de mogelijkheden in kaart; - coördineert het traject van aanmelding en wachtlijst tot plaatsing of overdracht; - organiseert bezichtigingen en maakt afspraken over de dienstverlening; - werkt nauw samen met onder andere huisartsen, ziekenhuizen, zorgaanbieders, locatiemanagers, artsen en coördinerend verpleegkundigen; - bewaakt wachtlijsten en beschikbare plaatsen en draagt bij aan een goede benutting van onze zorgcapaciteit. Je krijgt te maken met verschillende vormen van ouderenzorg. Het Klantcentrum organiseert de instroom van cliënten voor onze woonzorglocaties en de in- en uitstroom voor revalidatie en herstelzorg. Daarnaast verzorgen de Klantadviseurs de verhuur van verschillende zorgwoningen. Je komt terecht in een team van acht medewerkers, waaronder een teamcoördinator. Je werkt vooral vanuit het servicebureau van Amaris in Laren. Voor een bezichtiging of overleg bezoek je af en toe een van onze zorgloc...
