In het kort: Je gaat inwoners met een Banenafspraak begeleiden naar duurzame arbeid, trajecten vormgeven en bijstellen, bemiddelen tussen klant en werkgever, intensief samenwerken met partners en zorgen voor zorgvuldige rapportage en dossiervorming. Organisatie Eemkracht biedt ondersteuning aan inwoners van de drie gemeenten Baarn, Bunschoten en Soest. Eemkracht voert een breed palet aan taken uit, van werk en inkomen en Wmo-voorzieningen tot begeleiding, jeugdhulp, leerplicht en leerlingenvervoer. We werken nauw samen met de betrokken gemeenten en lokale welzijnspartijen, waarbij de hulpvraag van de inwoners centraal staat om zo passende hulp te bieden. Hierbij staan onze kernwaarden centraal: Betrokken, Samen en Professioneel. Als uitvoeringsorganisatie (een gemeenschappelijke regeling, GR) werken we voor een diverse samenleving. Daarom vinden we het belangrijk dat onze organisatie net zo divers is. Opdracht Ter vervanging van een zieke medewerker zijn wij opzoek naar een Klantmanager Banenafspraak. De opdrachtnemer is verantwoordelijk voor de uitvoering van de werkzaamheden behorend bij de functie van Klantmanager Banenafspraak. De werkzaamheden bestaan onder andere uit: - Begeleiden van klanten met een indicatie Banenafspraak richting duurzame arbeidsdeelname; - Voeren van intake-, voortgangs- en begeleidingsgesprekken om de mogelijkheden, talenten en ontwikkelkansen van klanten te inventariseren en passende trajecten vorm te geven; - Opstellen, uitvoeren, monitoren en waar nodig bijstellen van individuele trajecten gericht op duurzame uitstroom naar werk; - Bemiddelen van klanten naar passende werkgevers en realiseren van duurzame plaatsingen, waarbij gebruik wordt gemaakt van de beschikbare re-integratievoorzieningen, waaronder doelgroepgerichte instrumenten zoals loonkostensubsidie; - Onderhouden van contacten en afstemmen met interne stakeholders, waaronder klantmanagers Inkomen, accountmanagers en medewerkers van het sociaal/lokaal team, ten behoeve van..
