Wij zijn op zoek naar een Klantmanager Jeugd, voor 36 uur per week. Als Klantmanager Jeugd behandel je meldingen en aanvragen voor jeugdhulp (ambulante jeugdhulp, deeltijdbehandeling, verblijf, etc.) van melding tot de beschikking. Ook onderhoud je contact met het Centrum voor Jeugd en Gezin, van waaruit veel hulpvragen gesignaleerd, doorgezet en soms toegekend worden. Daarnaast geef je voorlichting, advies en informatie op het gebied van de Jeugdwet en sta je inwoners met vragen op dit gebied te woord. Je verheldert en analyseert de ondersteuningsvraag en zet nader onderzoek naar de hulpvraag in gang. Je hebt contacten met zorgaanbieders over toewijzingen, mutaties, en andere vraagstukken. Ook heb je contact met de Sociale Verzekeringsbank over de Persoonsgebonden Budgetten. Daarnaast verricht je diverse administratieve handelingen in GWS4All Suite jeugd. Je houdt vinger aan de pols met betrekking tot de afhandeling van aanvragen, het opstellen van de beschikkingen, het voldoen aan de juiste regels, procedures en kwaliteitseisen en je houdt samen met de generalisten specialisten in de gaten wanneer er contact met de cliënt wenselijk of nodig is. Je stemt af met collega's, draagt zorg voor de uitvoering van afspraken met cliënten en bewaakt de voortgang van zowel interne als externe procedures.
