Wij zoeken een energieke en proactieve collega met oog voor het hele systeem rondom een inwoner. Iemand die met betrokkenheid en enthousiasme aansluit bij volwassenen, minderjarigen en gezinnen ondersteunt en hen ondersteunt om weer grip te krijgen op hun situatie. Als klantmanager Regie en Mandaat werk je met een brede doelgroep en vervul je een sleutelrol in het organiseren van passende ondersteuning. Jij bent de verbindende schakel tussen de inwoner en de betrokken hulpverleningsinstanties en externe partners en voert de regie over complexe hulpverleningstrajecten waarin vaak meerdere domeinen samenkomen, zoals jeugdhulp, Wmo, zorg, schuldhulpverlening en participatie. Daarbij zorg je ervoor dat de inwoner centraal blijft staan en dat ondersteuning integraal, doelgericht en op maat wordt ingezet. Wanneer de thuissituatie onveilig is, neem jij de regie. Je signaleert risico's en maakt deze bespreekbaar, schakelt tijdig de juiste partners in, bewaakt de veiligheid en stuurt actief bij waar nodig. Ook in deze situaties weet je overzicht en regie te houden en zorg je voor duidelijke afstemming tussen alle betrokkenen. Je werkt nauw samen met collega's binnen de gemeente, zorgaanbieders en externe partners en bewaakt de voortgang van het traject. Je werkt systemisch, kijkt verder dan de individuele hulpvraag/ondersteuningsvraag en weet wanneer je moet regisseren, verbinden of juist loslaten. Met jouw daadkracht, betrokkenheid en overzicht krijgen inwoners de ondersteuning die zij écht nodig hebben, ook in complexe of onveilige situatie.
