Klantmanager Werk Gemeente Functieomschrijving Klantmanager Werk Help jij inwoners om stappen te zetten richting werk en zelfstandigheid? Voor een gemeentelijke opdrachtgever in de regio Drechtsteden zoeken wij een Klantmanager Werk voor 32 uur per week. Je begeleidt inwoners met een afstand tot de arbeidsmarkt. Tijdens gesprekken breng je hun ervaring, mogelijkheden en belemmeringen in kaart. Samen stel je een haalbaar plan van aanpak op, gericht op betaald werk of een passende vorm van participatie. Je beheert je eigen caseload en houdt regie op de trajecten. Je motiveert inwoners, bewaakt afspraken en stuurt bij wanneer dat nodig is. Daarbij werk je samen met werkgevers, accountmanagers, re-integratiepartners en collega's binnen het sociaal domein. Je werkzaamheden bestaan onder andere uit: - Voeren van intake- en voortgangsgesprekken. - Opstellen en evalueren van individuele plannen van aanpak. - Inzetten en coördineren van passende re-integratietrajecten. - Signaleren van belemmeringen en inschakelen van aanvullende ondersteuning. - Informeren van inwoners over rechten en plichten binnen de Participatiewet. - Vastleggen van afspraken, voortgang en resultaten in het cliëntsysteem. Functieprofiel Klantmanager Werk - Je beschikt over een afgeronde relevante hbo-opleiding. - Je hebt aantoonbare gemeentelijke werkervaring als Klantmanager Werk of in een vergelijkbare functie. - Je hebt kennis van de Participatiewet en ervaring met re-integratie en participatie. - Je bent gewend zelfstandig een caseload te beheren en plannen van aanpak op te stellen. - Je communiceert duidelijk, motiveert inwoners en kunt waar nodig begrenzen. - Je werkt planmatig en zoekt actief de samenwerking met collega's en ketenpartners.
