Klantmanager werk & participatie Overheidsdiensten Functieomschrijving Klantmanager werk & participatie Help jij inwoners om stappen te zetten richting werk of deelname aan de samenleving? Voor een gemeente in Noord-Holland zoeken wij meerdere klantmanagers Werk en Participatie voor 36 uur per week. Je start zo snel mogelijk voor een periode van twaalf maanden. Je brengt de mogelijkheden en ontwikkelperspectieven van inwoners in kaart. Tijdens intakegesprekken onderzoek je wat iemand nodig heeft en welk traject aansluit: re-integratie, sociale activering of een andere vorm van participatie. Je motiveert inwoners, maakt concrete afspraken en begeleidt hen naar een passende arbeids- of participatieplek. Je werkzaamheden bestaan uit: - Voeren van intakegesprekken en beoordelen van de mogelijkheden van inwoners. - Opstellen, uitvoeren en bijstellen van trajectplannen. - Begeleiden en motiveren van inwoners met een afstand tot de arbeidsmarkt. - Inzetten en bewaken van passende re-integratie- en participatietrajecten. - Bewaken van de kwaliteit van het aanbod van re-integratiebedrijven en onderwijsinstellingen. - Vastleggen van voortgang en resultaten in Suite4WIZ en rapporteren aan de teammanager. Functieprofiel Klantmanager werk & participatie - Mbo+ werk- en denkniveau. - Minimaal twee jaar aantoonbare ervaring als klantmanager Re-integratie in de afgelopen vijf jaar. - Ervaring met inwoners met een afstand tot de arbeidsmarkt en sociale activering. - Bij voorkeur minimaal twee jaar ervaring als klantmanager of trajectbegeleider in de afgelopen drie jaar. - Ervaring met Suite4/Suite4WIZ is een pre. Je communiceert helder, werkt resultaatgericht en weet mensen te motiveren. Je hebt doorzettingsvermogen, handelt met tact en past jouw aanpak aan de situatie van de inwoner aan.
