Centraal staat de visie één gezin - één plan - één regisseur , waarbij de verbinding wordt gelegd tussen Wmo, Jeugdwet, Participatiewet en zorg en veiligheid. Je combineert het bestaande kwaliteitswerk met het ontwikkelen van een systeemgerichte werkwijze. Samen met medewerkers onderzoek je wat in de huidige praktijk goed werkt, waar knelpunten zitten en wat nodig is om de kwaliteit van ondersteuning aan het gezin als geheel te kunnen toetsen en verbeteren. Wat je gaat doen: - Modelleren en actueel houden van processen voor centrale triage, Sociaal Loket, Wmo, Jeugd en zorg en veiligheid in Engage Process Modeller; - Opstellen van duidelijke werkinstructies en procesplaten in samenwerking met de uitvoering; - Beheren en verder ontwikkelen van de kennisbank, inclusief inhoudseigenaarschap, actualisatiecyclus en versiebeheer; - Overnemen en voortzetten van het lopende kwaliteitswerk binnen het sociaal domein; - Ophalen bij medewerkers wat goed werkt in de huidige kwaliteitstoetsing en wat ontbreekt bij gezinnen met vragen op meerdere leefgebieden; - Vertalen van signalen uit de uitvoering naar concrete en bruikbare toetspunten; - Stapsgewijs ontwikkelen van een integrale toets die inzicht geeft in de situatie van het gezin als geheel; - Verbinden van de verschillende wetgebieden Wmo, Jeugdwet, Participatiewet en zorg en veiligheid binnen de nieuwe werkwijze; - Uitvoeren van dossier- en processteekproeven bij meervoudige casuïstiek, gericht op leren en verbeteren; - Signaleren van afspraken, systemen en routines die h...
