Jouw uitdagingen:

  • Je staat in voor het sociaal onderzoek bij en de verdere psychosociale begeleiding van gebruikers die zich in een zodanige medisch-sociale situatie bevinden, dat professionele ondersteuning en coördinatie van de thuiszorgsituatie zich opdringt.
  • Je geeft info aan de cliënten en je biedt hen hulp aan bij sociale wetgeving, administratieve, financiële en psychosociale problemen.
  • Je assisteert de verantwoordelijke in de begeleiding van het personeel van de verschillende diensten binnen de afdeling Zorg aan Huis.
  • Je organiseert regelmatig individuele werkbesprekingen die tot doel hebben het personeel te ondersteunen in hun taken, de hulp te evalueren en bij te sturen.
  • Je staat mee in voor de taakverdeling en het opstellen van uurroosters en vorming van de medewerkers.
  • Je werkt mee aan de optimalisering van bestaande of aan de uitwerking van nieuwe initiatieven ter ondersteuning van thuiszorg en mantelzorg.
  • Je werkt onder leiding van en rapporteert aan de verantwoordelijke zorg aan huis.

  • het diploma van master in het sociaal werk
  • het diploma van master in het sociaal werk en sociaal beleid
  • het diploma van master in de pedagogische wetenschappen, afstudeerrichting orthopedagogiek
  • het diploma van master in de agogische wetenschappen
  • het diploma van bachelor in het sociaal-agogisch werk met de titel van maatschappelijk assistent, of een daarmee gelijkgesteld diploma
  • het diploma van bachelor orthopedagogie
  • het diploma van bachelor sociale re-adaptatiewetenschappen
  • het diploma van bachelor gezinswetenschappen.
