Heb je ervaring binnen maatschappelijk werk en beschik je over een attest Hulpverlener BelRAI screener? Krijg je energie van rechtstreeks contact met burgers, huisbezoeken en het zoeken naar de juiste ondersteuning?

Bij SkillSourcing zetten we consultants in op projecten binnen lokale besturen, OCMW’s en organisaties met maatschappelijke impact. Afhankelijk van jouw ervaring en interesses word je ingezet op opdrachten waar jouw expertise het verschil maakt.

Momenteel zoeken we versterking voor een project binnen Dienst Maatschappelijk Werk rond BelRAI screenings en zorgbudgetten in regio Antwerpen. Hierna hebben we natuurlijk nog voldoende mogelijkheden binnen onze andere projecten.

Takenpakket:

  • Je gaat op huisbezoek bij aanvragers en brengt hun ondersteunings- en zorgnoden in kaart.
  • Je voert indicatiestellingen uit aan de hand van de BelRAI screener voor volwassenen en de BEL profielschaal voor jongeren.
  • Je informeert cliënten en mantelzorgers over zorgbudgetten binnen de Vlaamse Sociale Bescherming en mogelijke aanvullende sociale voordelen.
  • Je licht resultaten en beslissingen op een heldere, empathische en klantgerichte manier toe.
  • Je stelt dossiers zorgvuldig op en zorgt voor een correcte administratieve opvolging.
  • Je plant en organiseert jouw huisbezoeken zelfstandig en efficiënt.
  • Je neemt deel aan overleg- en intervisiemomenten en draagt actief bij aan kennisdeling binnen het team.

Wie zoeken wij?

  • Je behaalde een bachelor of master maatschappelijk werk (of een gelijkgesteld diploma)
  • Je beschikt over een attest BelRAI screener
  • Ervaring binnen maatschappelijk werk of een OCMW context in een sterke troef
  • Je bent ontzorgend, nauwkeurig en daadkrachtig en voelt je goed in een rol waarin je zelfstandig verantwoordelijkheid opneemt.
  • Je bent bereid je te verplaatsen naar projecten binnen een haalbare straal van ongeveer 1 uur reistijd, rekening houdend met de verkeerssituatie
