Voor het poliklinische orthopedie/amputatieteam zijn wij op zoek naar een enthousiaste en daadkrachtige MAATSCHAPPELIJK WERKER cluster Orthopedie/dwarslaesie 24-36 uur per week - De Hoogstraat biedt mogelijkheden voor kennisontwikkeling in een werkomgeving met veel deskundigheid over diverse diagnoses en behandelingen - Je werkt intensief samen binnen een multidisciplinair behandelteam; zoals psycholoog, ergotherapeut, fysiotherapeut, sporttherapeut en meer. - De Hoogstraat draagt bij aan de verbetering van de revalidatiebehandeling met wetenschappelijk onderzoek, innovatie- en implementatieprojecten. Wat ga je doen? Als maatschappelijk werker begeleid je revalidanten en hun naasten die te maken krijgen met de gevolgen van een dwarslaesie, een trauma, orthopedie of amputatie. Het accent van de huidige vacature ligt in de groep dagbehandeling revalidanten met een arm- of beenamputatie of trauma. Je ondersteunt de revalidant en diens naasten op psychosociaal vlak, geeft informatie en advies over praktische zaken en mogelijkheden in de thuissituatie en rapporteert zorgvuldig. Daarbij werk je intensief samen binnen een multidisciplinair behandelteam, waaronder een logopedist, psycholoog, fysiotherapeut, ergotherapeut, sporttherapeut en revalidatiearts. Je draagt daarmee actief bij aan de interdisciplinaire revalidatiebehandeling. Je stelt eigen begeleidingsplannen op en voert deze uit. Je evalueert, rapporteert en overlegt met betrokkenen. Je werkt zowel met revalidanten die klinisch zijn opgenomen, als met revalidanten die poliklinisch revalideren. We zien de afgelopen jaren verandering in de complexiteit van casuïstiek en systeemproblematiek. Wij zoeken daarom iemand die daadkrachtig en doortastend is en die, samen met je collega maatschappelijk werkers, de rol van maatschappelijk werk hierop blijft aanpassen. Na...
