Voor de sociale dienst ZAS Psychiatrisch Ziekenhuis Stuivenberg zijn we op zoek naar een tijdelijke gedreven, flexibele, enthousiaste collega. Je komt terecht in een dynamisch, multidisciplinair team. Je zal verantwoordelijk zijn voor de patiënten op Verslavingszorg. WAT?

  • Vervangingscontract (in kader van ziekte van één van onze medewerkers)
  • ZAS Psychiatrisch Ziekenhuis Stuivenberg (2060, Antwerpen)
  • 38 uur/week (100%)

OVER DE JOB

  • Sociaal onderzoek – Je voert een grondige analyse uit van de sociale situatie van de patiënt en bepaalt welke ondersteuning het best aansluit bij hun noden.
  • Ontslagbegeleiding – Je begeleidt patiënten en hun omgeving naar een haalbare en veilige ontslagsituatie, rekening houdend met zowel thuissituatie als ziekenhuiscontext.
  • Psychosociale ondersteuning – Je werkt nauw samen met het multidisciplinair team om het psychosociaal welzijn van patiënten en hun omgeving te versterken.
  • Methodisch werken – Je past diverse begeleidingsmethodieken toe, zoals individuele gesprekken, om patiënten op maat te ondersteunen.
  • Registratie & opvolging – Je registreert al je interventies (zoals ontslagbegeleiding, type hulpvragen, doorverwijzers) om de interne werking en patiëntopvolging te optimaliseren.
  • Netwerk & samenwerking – Je bouwt een sterk netwerk uit met externe partners, coördineert acties in individuele dossiers en zorgt voor vlotte doorverwijzingen.

OVER JOU

  • Diploma – Je hebt een bachelor sociaal werk of sociaal verpleegkunde en kent het hulpverleningslandschap door en door.
  • Digitale vaardigheden – Je werkt vlot met Microsoft Office en leert nieuwe informaticatoepassingen snel aan.
  • Kennis van regelgeving – Je bent goed op de hoogte van sociale voorzieningen, rechtenonderzoek, sociale kaart en wetgeving.
  • Communicatief sterk – Je communiceert helder en empathisch in het Nederlands en hebt sterke interculturele en administratieve vaardigheden.
  • Zelfstandig & gestructureerd – Je werkt autonoom, stelt prioriteiten en behoudt overzicht in complexe situaties.
  • Probleemoplossend – Je denkt mee, zoekt oplossingen en schakelt flexibel tussen patiënt, netwerk en zorgteam.
