Jij bent samen met je collega's verantwoordelijk voor het totale pakket aan zorg bij de Prettig Thuis cliënten. Omdat je als MBO Verpleegkundige een voorbeeld bent, ben je zichtbaar bij de cliënten en voer je alle werkzaamheden uit van persoonlijke zorg tot verpleegkundige handelingen. Daarnaast coach en begeleid je je collega's. Binnen Prettig Thuis ga je aan de slag om de kwaliteit van de zorg en welzijn te verhogen en de processen te verbeteren. Je zorgt dat processen rondom de cliënt optimaal verlopen en je initieert en implementeert procesverbeteringen. Tevens draag je zorg voor de benodigde randvoorwaarden, bewaakt de kwaliteit van zorg- en dienstverlening en stuurt bij waar nodig. Je stimuleert, motiveert en faciliteert teamleden, bevordert het zelf organiserend vermogen in de teams en vergroot de taakvolwassenheid. Je maakt gebruik van aanwezige kennis en competenties en zorgt samen met de teams dat deze op peil blijven. Ook zorg je voor een effectieve communicatie- en overlegstructuur en voor verbinding tussen teams binnen en buiten Zorgbalans. Je stimuleert kennisdeling en bewerkstelligt een lerende sfeer binnen de teams. Werken als MBOVerpleegkundige in de wijkzorg bij Zorgbalans betekent dat je werkt in een klein, zelforganiserend buurtteam. Je werkt met een vaste groep collega's en cliënten. Samen met je cliënt bepaal je welke zorg hij of zij nodig heeft. Zo lever je zorg op maat en bouw je een (h)echte band op.
