MBO-verpleegkundige Overbruggingszorg (ELV) - Hengelo - 24 uur Jij krijgt energie van de kortdurende zorg. Stel Aniko je vraag Aniko Gering Leidinggevende Zorg Waar hebben we plek? Wij zijn op zoek naar een mbo-verpleegkundige binnen de overbruggingszorg die met positieve energie en een actieve werkmentaliteit een voorbeeldrol kan pakken in het team. Je schakelt makkelijk en verleent met aandacht de juiste zorg. Tijdens jouw verantwoordelijke dienst ben je niet alleen voor je eigen team, maar ook voor de locatie het eerste aanspreekpunt. De tijdelijke zorg voor cliënten op de overbruggingsafdeling geven dynamiek en afwisseling en zijn de reden dat geen dag hetzelfde is. De afdeling bestaat uit 24 bedden waarvan 14 bedden voor ELV-indicatie zijn. De overige bedden zijn bestemd voor cliënten met een WLZ-indicatie die wachten op een plek binnen een verpleeghuis. We hebben momenteel plek binnen de ELV kant. Wat ga jij doen? Zorg verlenen en regelen Als MBO verpleegkundige zorg jij voor de beste ondersteuning van de cliënt waarbij kwaliteit van leven altijd voorop staat. Jij verzamelt de benodigde informatie door middel van klinisch redeneren en triage om de juiste verpleegkundige diagnose te stellen. Bij de dagelijkse verzorging, verpleging en begeleiding kijk je hierbij naar de wensen en behoeften van de cliënt en staat de eigen regie voorop. Jij richt je op de coördinatie en continuïteit van zorg en het bewaken hiervan. Jij weet wat de cliënt nodig heeft en handelt daarnaar. Naast het leveren van kwalitatieve zorg regel jij dat het zorgdossier wordt opgesteld en voer jij verpleegtechnische handelingen zelfstandig uit. In gesprek De dialoog met de cliënt loopt als een rode draad door jouw werk. Jij bespreekt continu de veranderingen in de gezondheidssituatie. Daarbij verlies je het netwerk rondom de cliënt niet uit het oog. Goed contact met het netwerk rond de cliënt staat dus voorop. In onze 'teams voor persoonlijke zorg'...
