Mbo-verpleegkundige PG - Vriezenveen - 24 uur Jij bent van samenwerken. Stel Franciska je vraag Franciska Willems Leidinggevende zorg Waar hebben we plek? We zoeken voor pg-afdeling Leemansmolen van de Weemelanden een collega die invulling geeft aan samenwerken en toekomstgerichte zorg. We bieden een leuke, afwisselende baan, waar geen dag hetzelfde is. Op deze afdeling wonen 10 bewoners en werk je samen met een betrokken team van ongeveer 12 collega's. Samen zorgen jullie voor een vertrouwde, veilige omgeving waarin cliënten zich gezien voelen. Je werkt nauw samen met de naastgelegen PG-afdeling en schakelt gemakkelijk met naasten, vrijwilligers en andere disciplines. Tijdens je verantwoordelijke dienst (EAV) ben je het aanspreekpunt voor de hele locatie. In de nachtdienst werk je samen met de collega's van locatie 't Haarhuus in Westerhaar. Wat ga jij doen? Verpleegkundige zorg Als mbo-verpleegkundige verleen jij zelfstandig verpleegkundige zorg aan cliënten met complexe zorgvragen. Je schakelt continue tussen verschillende rollen: van zorgverlener en coördinator tot coach en aanspreekpunt. Daarbij staat de kwaliteit van leven van de cliënten altijd voorop. Jij denkt mee Jij informeert, adviseert en instrueert de client en diens netwerk over de gezondheidssituatie. Goed contact met het netwerk rond jouw bewoners staat dus voorop. In onze 'teams voor persoonlijke zorg' hebben namelijk ook naasten een belangrijke rol. Je betrekt de juiste collega's en disciplines bij de zorg en denkt mee over passende oplossingen voor cliënten en hun naasten. Jij verbindt Jij stemt af met collega's uit verschillende disciplines, draagt informatie zorgvuldig over en zorgt dat cliënten de juiste zorg krijgen.
