Hierdoor kunnen zij langer thuis wonen. Dat is: Jij en Woonzorg Flevoland. Jij en jouw werk: Voor onze dagbesteding locatie Bruis in Almere zoeken wij een nieuwe collega. Op onze dagbesteding locatie voor mensen met dementie staat het bieden van een zinvolle dag centraal. We werken vanuit het gedachtegoed belevingsgerichte zorg, waarbij we aansluiten bij wat de deelnemer nog wél kan en nodig heeft. Het doel is het bevorderen van de zelfredzaamheid en tegelijkertijd het ontzorgen van de thuissituatie. Dementie kent een veranderend ziektebeeld daarom kijken we steeds opnieuw naar de behoeften en beleving van de deelnemer zodat we passende activiteiten en ondersteuning kunnen blijven bieden die bijdragen aan de kwaliteit van leven. Daarnaast: - Werk je nauw samen met vrijwilligers en stagiaires; - Werk je samen met andere disciplines binnen de keten en andere organisaties binnen het netwerk om ontmoetingen te faciliteren en verbindingen te leggen; - Onderhoud je actief contact met mantelzorgers om een sterk netwerk rondom de deelnemer te creëren; - Begeleid je kennismakingsgesprekken en rondleidingen met toekomstige deelnemers en hun mantelzorger; - Voer je intakegesprekken met nieuwe deelnemers en hun mantelzorger. Zo zorgen we voor jou: - Een salaris volgens de CAO VVT FWG 35; tussen de € 2.735 en € 3.592 bruto per maand op basis van 36 uur per week; - Goede secundaire arbeidsvoorwaarden zoals: 8% vakantiegeld, eindejaarsuitkering van 8,33% en een reiskostenruil...
