Deze vacature kadert binnen de oproep vanuit de FOD om de werking van een mobiel team acute zorg uit te breiden naar urgentiepsychiatrie

Het Mobiel Team Acute Zorg is een ambulant multidisciplinair team dat zich richt op volwassenen en ouderen (vanaf 18 jaar) met acute psychiatrische problemen in de thuissituatie. Het doel van crisisinterventie aan huis is het voorkomen van een opname en het vermijden van herval. Het team ondersteunt personen die gehuisvest zijn in Beveren-Kruibeke-Zwijndrecht, Lokeren, Moerbeke, Sint-Gillis, Sint-Niklaas, Stekene, Temse, Waasmunster, Hamme, Zele, Berlare, Dendermonde, Buggenhout en Lebbeke. Het MTAZ maakt deel uit van het netwerk Geestelijke Gezondheidszorg Aalst Dendermonde Sint-Niklaas (netwerk GGADS).

Je start binnen de huidige werking van het mobiele crisisteam. Van hieruit kan er snel geëvolueerd worden, zowel inhoudelijk als organisatorisch, naar de urgentiewerking. Je werkt in een rooster met dag- en late shiften en weekendwerk. Op langere termijn is er een verdere uitrol naar bereikbaarheid gedurende de nacht.

  • Je hebt een bachelor of HBO5 diploma in de verpleegkunde of bent afgestudeerd als bachelor in een andere menswetenschappelijke richting (orthopedagogie, ergotherapie,…).
  • Je hebt kennis en ervaring in het werken met cliënten met een psychiatrische kwetsbaarheid.
  • Je bent vaardig in het ageren in crisissituaties en bent besluitvaardig in het zoeken naar oplossingen.
  • Je hebt kennis van de sociale kaart in het netwerk GGZ ADS.
  • Je beschikt over de nodige organisatorische vaardigheden (agenda kunnen beheren, planmatig werken, flexibel handelen bij onverwachte situaties,…).
  • Je hebt aandacht voor de belevingswereld van de cliënten en hun familieleden. Naasten worden als volwaardige partners in de zorg betrokken.
  • Je stelt begrippen als cliëntparticipatie en empowerment voorop vanuit een herstel ondersteunende visie.
  • Je stelt persoonsgerichte zorg en presentie centraal in je dagdagelijks handelen.
