We zijn een dynamisch sociale organisatie in volle groei en met een sterke visie en zoeken voor onze afdeling mobiele ondersteuning een mobiele begeleider voor ons volwassenenteam in Noorderkempen. Je uitvalsbasis is in Brasschaat en Wuustwezel (Gooreind).

Kernopdracht

Elke dag opnieuw geef je het beste van jezelf. Cliënten en collega’s laat je groeien en bloeien, en mocht het wel eens tegenzitten dan blijf je communiceren. Jouw open houding is daarom heel belangrijk in deze job.

Je biedt in netwerkverband psychosociale ondersteuning aan zelfstandig wonende volwassen cliënten met een beperking. Sommige cliënten wonen met enkelen samen in een gebouw van OLO-Rotonde en hebben daar hun eigen studio. Je begeleidt hen individueel, maar ondersteunt waar nodig ook de groepsdynamieken die er zijn.

De intensiteit van de begeleiding wordt op maat van de cliënt en zijn netwerk afgesproken. De verschillende levensdomeinen kunnen aan bod komen. Je plant en organiseert de begeleidingen zelfstandig.

Je organiseert en coördineert samen met cliënt de ondersteuningsbronnen die nodig zijn zodat de cliënt zijn inclusief levensdoel kan bereiken. Je werkt dus nauw samen met het netwerk van de cliënten. Samen met cliënten en partners denk je na over hoe een warme inclusieve buurt voor iedereen gecreëerd kan worden.

  • Je durft zelfstandig aan de slag te gaan en vindt het een uitdaging om de buurt en cliënten met elkaar te verbinden.
  • Je neemt zelf initiatief en gaat enthousiast en gedreven aan de slag.
  • Je bezit (ortho-agogische) kennis en hebt bij voorkeur ervaring in het begeleiden van volwassenen met een beperking.
  • Je beschikt over voldoende probleemoplossend vermogen om het verhaal van de cliënt samen te kunnen waarmaken.
  • Je bent bereid om in supervisie over jezelf na te denken en met je werkpunten aan de slag te gaan.
  • Je kan op een constructieve manier samenwerken, zowel met collega’s als met andere diensten en met het netwerk van de cliënt.
  • Je wil met de sociale kaart en sociale wetgeving aan de slag.
  • Je woont bij voorkeur in de regio Noorderkempen.
  • Je bent in het bezit van een eigen wagen en rijbewijs B.
  • Je bent bereid om volgens een flexibel uurrooster te werken (zonder echt een shiftensysteem te hebben). Je vindt het niet erg om af en toe op zondag en ’s avonds te werken.

Diploma

  • Bachelor met pedagogische, sociale of paramedische oriëntatie.
