Vanuit het principe van kwaliteit van leven bieden we ondersteuning en houvast aan mensen en hun omgeving, waarbij wensen, noden en verwachtingen steeds centraal staan. Momenteel zijn wij op zoek naar een mobiele begeleider in de regio Rivierenland (Lier, Dijle- en Neteland).

Wat ga je doen?

Als mobiele begeleider ben je een compagnon de route: tijdens individuele begeleidingsmomenten luister je naar de vragen van de cliënt binnen hun eigen leefomgeving. Je coacht, helpt mee plannen, coördineert, contacteert andere mensen of instanties, volgt mee afspraken op.

Je ondersteunt cliënten op verschillende levensdomeinen van kwaliteit van leven. Samen ga je op zoek naar een betekenisvol traject rond onder andere wonen, werk, dagbesteding, netwerk, familie en begeleiding. Je helpt mensen om een nieuw evenwicht te vinden in hun leven door informatie, advies en ondersteuning op maat aan te bieden.

Hoe een traject van iemand eruit ziet, is afhankelijk van de persoon en zijn dromen, wensen en doelen.

Wie ben je?

  • Je kan makkelijk contacten met anderen leggen waardoor je goed overweg kan met de cliënt en zijn/haar netwerk.
  • Je kan omgaan met complexe situaties en behoudt rust en professionaliteit, ook wanneer uitdagingen zich voordoen.
  • Je kan je flexibel en ruimdenkend Hierdoor kan inspelen op veranderende noden en schakel je vlot tussen verschillende contexten en hulpvragen.
  • Je kan je inleven in de leefwereld, wensen en noden van cliënten. Dit is essentieel om ondersteuning op maat te bieden en een vertrouwensband op te bouwen
  • Als mobiele begeleider werk je vaak zelfstandig en neem je initiatief in het begeleiden van trajecten vertrekkende vanuit empowerment. Een goede organisatie en verantwoordelijkheidszin zijn daarom belangrijk.
  • Je hebt een bachelor-diploma in een sociaal-agogische richting en je hebt al enkele jaren werkervaring (vb. contextbegeleiding, leefgroep … ). Ervaring in het werken met volwassenen met een niet aangeboren hersenletsel is een meerwaarde.
  • Je beschikt over een rijbewijs B en een wagen.
  • Je begeleidt mensen in de ruime regio Rivierenland (Lier, Dijle- en Neteland) en bent hier woonachtig.
