Tijdelijke vacature Onafhankelijke cliëntondersteuner (OCO) 16 uur per week In verband met een ziekte vervanging zijn we met spoed op zoek naar een tijdelijk onafhankelijk clientondersteuner voor Zuidplas en Gouda. Stichting KernKracht is een zelfregie, herstel en belangenorganisatie ter bevordering van psychische, sociale en maatschappelijke gezondheid in de regio Midden-Holland. Taken/verantwoordelijkheden onafhankelijk cliëntondersteuner: Ondersteunt altijd vanuit cliëntperspectief, dat betekent dat het belang van cliënten altijd voorop staat: • Ondersteunt cliënten bij het in kaart brengen van hun vraag en behoefte. • Versterkt de eigen regie van cliënten. • Adviseert en ondersteunt bij het vinden van de weg in het sociaal domein. • Bereidt de cliënt en zijn relaties voor op het voeren van het gesprek met de zorgaanbieder over de invulling van de zorg. • Ondersteunt bij het opstellen, evalueren en bijstellen van het ondersteuningsplan en bij zorgbesprekingen. • Ondersteunt bij het kiezen van een passende zorgaanbieder. • Bemiddelt ten behoeve van de cliënt indien zich knelpunten in de zorg voordoen. • Ondersteunt zo nodig bij een formele klacht-, bezwaar-, of beroepsprocedure. • Ondersteunt familieleden en/of mantelzorgers. • Herkent signalen en trends in de knelpunten in de zorg en is in staat deze goed schriftelijk vast te leggen • Registreert in het registratiesysteem Central Station Algemeen • Actieve bijdrage leveren in relevante netwerken (zowel in- al extern) • Stimulerend samenwerken met de ketenpartners Wij vragen Je hebt een MBO+/HBO werk- en denkniveau en liefst werkervaring op het gebied van OCO en samenwerken met vrijwilligers. Werken vanuit cliëntenperspectief is voor jou een vanzelfsprekendheid. Je hebt kennis van de begrippen herstel, empowerment en ervaringsdeskundigheid.
